- Include copies of updated ACRS/APARs for the last five years, duly attested by an officer of the level of Under Secretary or above.
- Attach vigilance clearance and integrity certificate.
- The Cadre controlling authority must verify and certify the correctness of the furnished particulars and confirm no major or minor penalty was imposed on the officer during the last 10 years of service.
